Customers who have consented to receive their tax forms electronically will be able to view and
download their income tax forms directly from the FTX portal.
Steps to download your tax form:
1. Log into your account on the FTX Claims Portal: https://claims.ftx.com/welcome
2. Click on the “Income Tax Forms” section located at the upper right corner of the page.
3. Click the download button next to each available tax form listed (ex. 1099B, 1099-INT,
1099-OID, or 1042-S).

If you are unable to see the “Income Tax Forms” section, please try logging out and then logging back into your account, or clear your browser's cache.
If you have not previously consented to electronic delivery of your tax documents, you will not be able to receive tax documents through the FTX Claims Portal. Instead, your tax documents will be mailed to the address listed on your submitted tax form (Step 7).
